The following forms are required on your first visit to Sound Acupuncture. Filling them out ahead of time will allow your treatment to begin as quickly as possible.
Clicking on each of the links will allow you to download the Microsoft Word document or the PDF file. Please either email them back to jessicamartenslac@gmail.com or print the Health Intake and Receipt of Privacy Notice forms and bring them to your first appointment. You do not need to bring a copy of the HIPAA Privacy Notice; that form is for your records. If you choose the PDF files, please note that you cannot fill them out on the computer, they must be printed.
